Fibromyalgia is a widespread chronic musculoskeletal pain disorder that involves at least six different sites in the body. It is accompanied by fatigue, disrupted sleep, and problems with thinking and mood. Affected muscles, ligaments, and tendons are tender, but patients otherwise look normal, without signs of inflammation. Labs and radiologic studies are normal.
The lack of objective findings has led some to question the existence of the disorder. Even among physicians, the diagnosis tends to be missed or downplayed. As a result, many patients often feel frustrated and abandoned. Brain image studies, however, suggest the disorder is real, perhaps due to a disturbance of central nervous system pain processing.
Fibromyalgia is found in 2-3% of the population. It is the most common cause of widespread musculoskeletal pain in women between 20 and 55 years of age. It occurs in women seven times more than in men. Patients may also have other conditions, such as irritable bowel syndrome and migraine headaches. Fibromyalgia can be worsened by arthritis, obstructive sleep apnea, or restless leg syndrome.
Treatment of fibromyalgia can be challenging. A multidisciplinary approach is ideally used, including patient education, exercise/movement/physical therapy, counseling (e.g., cognitive behavioral therapy), and prescribed medications (e.g., tricyclic antidepressants and others). Unfortunately, medications produce only a moderate benefit and only in a minority of patients. Most patients eventually stop their medications due to lack of benefit and side effects. Many patients end up with chronic pain, fatigue, disturbed sleep, anxiety, and depression.
The studies supporting the benefits of ketamine for fibromyalgia mainly describe short-term benefits shortly after infusion. These encouraging findings offer new hope that ketamine can improve the lives of those suffering from fibromyalgia.
